Every espresso machine has to solve the same physics problem: brewing wants water at roughly 90–96°C, steaming milk wants steam at roughly 120–130°C (about 1–2 bar of boiler pressure). One temperature for coffee, a much higher one for milk. How a machine handles that conflict is the single biggest thing you're paying for, and it splits the entire market into three designs.
Single boiler: one temperature at a time
One boiler does both jobs — just not at once. You pull your shot at brew temperature, then flick to steam mode and wait (typically 30 seconds to a few minutes depending on boiler mass) while the same boiler climbs 30-odd degrees. Steam first and you have to bleed the temperature back down before brewing, or you'll scorch the shot.
The honest case for it: it's how you test the waters of espresso-based drinks without over-committing. For one or two milk drinks a morning the wait is a mild annoyance, not a dealbreaker — and single boilers with PID control, like the Lelit Anna PID (R14,495) or Lelit Victoria (R18,995), brew every bit as accurately as machines costing three times more. They just can't do two things at once.
Budget: expect a full setup — machine plus grinder — under R20k.
Heat exchanger (HX): one boiler, two temperatures, simultaneously
The clever middle path. The boiler runs hot at steam temperature full-time, and brew water passes through a coiled pipe — the heat exchanger — running through that boiler, picking up brew temperature on the way to the group. Result: steam while you pull shots, from one boiler. It's the design that let café machines serve continuously since the 1960s, and it's why the E61 group head exists.
The honest case for it: it's the smart way to save money and still get simultaneous brew and steam. The classic HX trade-off is brew-temperature drift (managed with cooling flushes), but modern implementations have largely engineered that away — the Lelit Mara X (R30,995) actively manages exchanger temperature so well it behaves nearly like a dual boiler, and the hand-built Rocket Appartamento (R40,503) has been many people's forever machine. Both are great — they're the two we name first in this category.
Budget: everything in between — realistically R30k–R45k for machine plus grinder.
Dual boiler: two boilers, zero compromise
One boiler dedicated to brewing, held at brew temperature by PID, and a separate steam boiler held at steam pressure. Each does one job at one temperature, all day. This is always the ideal configuration: the most stable brew temperature regulation of the three designs — typically holding setpoint within a degree — plus stronger steam pressure than an HX, because the steam boiler is built and sized purely for steam.
The honest case for it: if the budget reaches, this is the answer. Back-to-back flat whites for a houseful of guests without the machine falling behind. The Petrus PE3922 Max (R30,400) is the value entry — stainless boilers, rotary pump, volumetric dosing. The Lelit Bianca V3 (R54,995) adds flow profiling and is the machine on our own counter.
Budget: R40k-plus for a full setup, with the Petrus the notable exception that undercuts the rule.

The verdict
Dual boiler is always ideal — the physics is simply better. A heat exchanger is the intelligent compromise, and machines like the Mara X and Appartamento prove how little you actually give up.
